Mana damage reduces a player character's mana rather than life. If the monster is not at maximum life, healing equivalent to final mana damage to the character is applied. No other units have mana, so healing only applies when attacking player character and it is limited by current mana: a character with no mana takes no mana damage, and thus no healing is applied.
