- Ladder is a game mode on the Realms in which characters compete in a race to level 99.
- Prior to the release of patch 1.10 on 28 October 2003, all characters were automatically ranked on a Ladder according to experience level (although there were separate Ladder rankings for different Realms, for Hardcore characters on each Realm, and later for Classic and Expansion characters on each Realm). When Ladder was reset, existing characters were removed from the rankings and new characters had to be created to participate in the new Ladder race.

- However, with the release of 1.10, Ladder also became a separate game mode.
- Ladder characters are created on the Realms by checking the Ladder Character box when creating a new character. Ladder characters can only create and join Ladder games, which non-Ladder characters cannot join. Once a Ladder character has been created, it cannot be converted to a non-Ladder character until the end of the Ladder season.
- Ladder rank of a dead Hardcore character will be preserved as long as it is not deleted. Names of dead characters can be recycled by deleting them and creating new characters with the same name (although any Ladder rank of the dead character will be lost). If a player does not log in with a dead Hardcore character at least once every 90 days, it expires and can then be deleted by anybody else creating a new character with the same name.
- Deleted Ladder characters are removed from the Ladder rankings.
- Ladder is periodically reset, at which point previous Ladder characters are converted to non-Ladder characters. To participate in a new Ladder season, new Ladder characters need to be created. All new Ladder characters start at level 1 with no items, so the economy is also reset.
- Aside from the Ladder competition and fresh start, there are Horadric Cube recipes, Rune Words and Unique items available in Ladder games which are not available in non-Ladder games on the Realms (although previous Ladder characters retain their Ladder items when converted to non-Ladder at the end of a season).
- Ladder games also present more of a challenge:
- Base monster defense and life is higher in Ladder games.
- Base monster damage and poison length calculated using MonStats values increases by 1/16 (6.25%) with each additional player in Nightmare and Hell Ladder games.

- Ladder Rune Words were released server-side at the beginning of season 2 on 8 July 2004. However, they weren't published on the Arreat Summit until 10 December 2004. [13]
