Target overlay when poison damage is applied.
- Targets turn green when poisoned, a state which stops damage regeneration and can apply poison damage to the target every frame of the state's length (unless cold length or Holy Freeze are also applied, in which case they remain or turn blue). While damage is displayed in points and length in seconds, both are calculated and applied using more precise units of measurement: damage is calculated and applied in bits, and length in frames. [1]

- Targets are poisoned if as little as 1 bit poison damage is applied per frame.
- Poison damage is displayed as the total damage applied over its length, but it's calculated by applying a bit rate (bit damage per frame) for a specified number of frames: a Toxic Small Charm applies 205 bit rate for 125 frames, which is displayed as 100 Poison Damage Over 5 Seconds (205*125/256 over 125/25).
- Poison damage from multiple attacks doesn't sum: if a target is already poisoned, then the rate and length of that poison will be overwritten by that of any subsequent attack which has an equal or greater rate. Rates are compared after all modifiers have been applied. [2]
- When player, mercenary or pet applies poison to a poisoned monster, their poison is applied for one frame instead of the current poison: if their poison has an equal or greater rate, it then overwrites the current poison and it gets applied for its length; if their poison has a lower rate, the current poison then continues for its remaining length. Rates are compared after all modifiers have been applied. More generally, poison damage does not get applied in frames in which any damage from other sources is applied (although poison and Open Wounds both apply simultaneously). [3] [4]
- When using Whirlwind with two weapons with the same attack rate against a single target, which can result in poison damage being applied by both attacks in the same frame, each attack applies one frame of its poison damage, then the highest rate is applied for its remaining length. When equal rates are applied, the length of the last is applied (whichever is the secondary weapon for that attack check). [5]
- When monsters apply poison to a player, mercenary or pet, their poison is applied for one frame as well as the current poison: if their poison has an equal or greater rate, it then overwrites the current poison and it gets applied for its length; if their poison has a lower rate, the current poison then continues for its remaining length. Rabies and Poison Creeper can also apply additional frames of poison damage when applied to more than one monster.
- Poison damage can kill monsters, mercenaries and pets but will only reduce players to 1 life (although poison damage from a subsequent attack would kill them if they remained on 1 hit life). [6]

- LR is applied when the target is poisoned, so it can be overwritten by another curse immediately afterwards: even if LR is required to break immunity, once poison has been applied its rate and length remain unchanged, even if the monster becomes Immune to Poison again. [16]
-% Enemy Poison Resistance
- -% Enemy Poison Resistance reduces a target's Poison Resist like Lower Resist, but only if PR < 100% i.e. not Immune to Poison. However, unlike LR -% EPR also applies to length, effectively applying Poison Length Reduced by -%. [17]
- Monsters have no PLR % so -% EPR increases poison length by the displayed amount: PLR -100% is the floor. The only exceptions are the Ancients, whose equipment can spawn with PLR % suffixes: Korlic and Talic can spawn with PLR >99%, which always reduces poison length to a single frame. [18]
- Player and mercenary have a PLR 75% ceiling, although the same difficulty level penalties that apply to resistance apply (-40% in Nightmare and -100% in Hell). [19]
- Like LR, -% EPR is applied when the target is poisoned. Once poison has been applied its rate and length remain unchanged, so -% EPR can be removed afterwards without affecting damage or length. [20]
- 25,022-26,317 bit rate for 350 frames results in 34,209-35,980 poison damage being applied over 14 seconds to a PR 0% target. Casting level 10 LR (PR -55%) and equipping Trang-Oul's Wing Cantor Trophy with two other items from the set to apply its -25% EPR partial set bonus results in the following:

- Ideally damage is calculated measuring rate in bits and length in frames. It's also possible to calculate damage measuring rate in poison damage/second and length in seconds, which can be derived directly from displayed damage. However, the result will only be an approximation and rounding down should only be done at the end of the calculation for maximum accuracy.
- If a single item has both a prefix and a suffix applying poison damage, then the rates and lengths of both are simply added.
- Gems, jewels and runes all count as separate items, even if they're socketed in a single item.
- Item and Venom rate are only added to the skill rates of Poison Dagger and the javelins (but not the clouds) of Poison Javelin and Plague Javelin.
- +% PSD isn't applied to item and Venom rate when applied by ranged attacks.
- Length is calculated differently when multiple items applying poison damage are used with skills that apply less than 100% weapon damage.
